Taraba Advocacy and Rehabilitation (TARe) Foundation is an independent development and humanitarian non-profit organization established in 2003 committed to enhancing the quality of lives of the vulnerable population including women, children, and key populations (KP) in Nigeria. We provide locally driven solutions to advance better health, education, and other social services development outcomes.  TARe Foundation is registered in Nigeria with the Corporate Affairs Commission.

TARe in partnership with AHNI will implement a six months national Aligned HIV/AIDS Initiative (NAHI) project. The project aims to promote the acceleration of high-quality HIV prevention, care, treatment, and support services for KPs (FSW) programs in Taraba State.

We are recruiting to fill the position below:

 

Job Title: Case Manager

Location: Taraba
Job Type: Full Time
Slot: 5 Openings

Responsibilities

  • Managing a portfolio of cases.
  • Establishing and maintaining effective relationships with key stakeholders.
  • Work with treatment facilities and establishments (One Stop Shops and/or CART teams) to initiate same-day, same-site commencement of Anti- Retroviral Therapy (ART) for positive KPs through accompanied referral and monitor adherence.
  • Provide Community-based PMTCT services/referral to a facility.
  • Provide Community-based viral load services (Sample collection, packaging, and transport) to an OSS/PCR laboratory.
  • Provide adherence support for HIV-positive KPs; follow up and report on retention in care as well as viral load suppression.
  • Ensure all clients have initiated on ARVs/Link all identified positives in the community to CART/OSS.
  • Provide/refer for clinical services including STI screening, treatment, and other sexual reproductive health services.

Knowledge and Experience

  • National Diploma (ND) or it's equivalent
  • Background in health care, HIV prevention & treatment services, and social work.
  • Excellent communication skills, both written and verbal.
  • Ability to speak local languages in Taraba state.
  • Minimum of 2 years experience managing operations and/or projects in developing countries, preferably in an emergency.
  • Experience as a case manager.
